Late Registration
Registration applications should be
submitted no later than May 14th. After that date, applications
will require an additional
$10 late fee,
per family. We will accept late registration until
August 24th
by mail. Please do submit your
registration as soon as possible. Late
registration is very time consuming to
process. Any registration received
after August 1st
is at risk of the player missing some games
because they may not receive their player
pass in time.
Option 2:
Mail Your Registration

Fill out a registration form. Fill out a
registration form. (see #3 below for specific
instructions). You may pick up a registration form at
the True Value Hardware store on Missouri Flat Rd. in
Diamond Springs or you may download the form from our
website by clicking here.

2. Items to mail: Please read these
instructions carefully, as any deviation
will delay the processing of your child's
application.

Photo copy of player's birth
certificate (ONE req'd for
recreation play, and TWO required
for select play). Must be issued
from the county of birth (not from
the hospital of birth). We are
required to collect a new copy of
the birth certificate each year.
NEW FOR
2008: IF the player was
registered with
High Sierra
Soccer as a U8 or
older player during the 2007
season, you do NOT have to
resubmit a Birth Certificate!

1.5" x 1.5" headshot of player being registered (no
sunglasses or hats
please). Important: the head/face
portion of this photo should occupy at least 1"x1" of
the photo space. Please do not deviate from these
specifications or we will be calling you for a new
picture, delaying your application.

A personal check, cashier's check,
or money order for
$100 made out to:
High Sierra
Soccer Club (be
sure to check out the multi child
discounts below, and late fee).

3. When filling out the registration
form:

hard for the copies. Or if you
downloaded the form, be sure to
provide the original and a copy.

Make
certain that all the information you
supply is correct.

Provide
the doctor's contact information.
(IMPORTANT!)

Please
provide at least 2 phone numbers.

Including
an e-mail address is highly
recommended, so that you can
receive soccer club news in a timely
manner.

Sign
the form.
Age Groups
Please refer to the
Age-Group Matrix to determine which
Age-Group your child will play in.
Multi-child discounts
The second child in the same family
is $85. The 3rd child is $80.
The fourth child is free.
Registration Refunds
Prior to August 1st, refunds can be
processed for a full refund. After August
1st but prior to September 1st, refunds will
be minus $35 League Fees that we submit to
begin team formation. After the first game of the season
(September 8th)
refunds are not available. NOTE: All
refunds will be processed at the first board
meeting after the first game of the season
is played.
Uniforms
Uniforms are for sale at each registration
event. Shirts are $20. Shorts
are $15. Socks are $5. A
complete set is $35.
